1.  Patients are expected to pay for the services they receive, in full at the time of service.

2.  We accept patient payments via cash, check, Master Card, Visa and Discover Card.

3. Cox Family Practice, PC does contract with most major insurance companies, with Medicare, and with Medicaid. Patients are welcome to call our office at 336-629-6500 to check for coverage.

4. Patients with medical insurance from a company, with whom Cox Family Practice, PC contracts, are responsible for either their entire co-payment, an estimate of their deductible or the co-insurance portion of their charges, on the day of service. Cox Family Practice, PC will file an insurance claim if we contract with the patient’s insurance company. Any remaining balance upon payment by the insurance is the patient’s personal responsibility and payment in full is due upon us sending a billing statement.

5. Patients with medical insurance from a company, with whom we do not contract, must also pay either their entire co-payment, an estimate of their deductible, or the co-insurance portion of their charges, on the day of service. As a courtesy, we will file an insurance claim with non-contracted insurance companies. However, if no response from the patient’s insurance company is received within 30 days of filing the insurance claim, the entire balance is the patient’s personal responsibility and payment in full from the patient is due upon us sending a billing statement.

6. A patient’s payment collected on the day of service is an estimate. If there is an additional balance due after the patient’s insurance company has paid their portion of the charges, then the entire remaining balance is the patient’s responsibility.  Payment in full is due from the patient upon sending a billing statement.

7. Patients without any insurance coverage are expected to pay for their entire charges in full on the day of service. By doing such they may be eligible for a reduction in some of their charges at the time of service.

8. Patients who do not pay their co-payment, deductible, or the co-insurance portion on the day of service, or patients with an overdue balance, may be unable to schedule appointments for their families until the balance is paid in full or until a satisfactory payment plan has been worked out (only exception – medical emergency).

9. Patients who do not respond to efforts to collect an overdue balance and establish appropriate payments may be turned over to a collection agency and dismissed from Cox Family Practice, PC. To avoid this action, we encourage patients with a balance to immediately pay their entire charges in full. Patients who are having financial difficulties should contact our office 629-6500 to establish a satisfactory payment plan rather than being dismissed and turned over to collections.
